Pennsylvania-based ThoroughCare, a digital care coordination company, has integrated into Canadian health tech company PointClickCare’s marketplace, allowing long-term and post-acute care PointClickCare providers to access ThoroughCare’s offerings in one place. 

ThoroughCare offers a platform aimed at addressing inefficiencies and fragmentation in patient care, which the company says can result in inconsistent patient engagement, poor outcomes and revenue loss. 

It supports a broad range of healthcare organizations, including independent practices, health systems, health plans and accountable care organizations, by helping manage medical, behavioral and social elements of care delivery. 

The Pittsburgh-based company provides digital care coordination to almost 700 care delivery organizations nationwide. 

PointClickCare is a cloud-based software company focused on elder care that covers the long-term and post-acute care market. It helps providers navigate transitions of care to collect patient information in one place, connect care teams with data, and connect care delivery and billing teams to improve the revenue cycle.

“By integrating our care management software with PointClickCare’s platform, we’re empowering providers with real-time data and actionable insights that enhance care delivery, improve chronic disease management, and ultimately drive better patient outcomes.”

THE LARGER TREND

In November, ThoroughCare closed a $5 million Series A funding round led by Empactful Capital, two years after raising $3 million in funding from Cypress Growth Capital.

Last year, PointClickCare acquired Patient Pattern, a value-based care electronic health record and integrated care management platform, which expanded the senior-focused company’s offerings to include a workflow management program and value-based care reimbursement assistance.

In 2022, the company announced a strategic collaboration with pharma giant Pfizer to share its real-world data on seniors. PointClickCare would provide Pfizer with real-world data insights that particularly represent older adults, especially those in long-term care facilities, to help guide decisions on new therapies.  

The company also launched a telemedicine service called Virtual Health. It offers skilled nursing facilities 24/7 access to hospitalists with the assistance of its partner Sound Physicians, a physician partner to health plans, physician groups, post-acute providers and hospitals.

PointClickCare has made several acquisitions, purchasing health IT company Audacious Inquiry in 2022, coordinated-care platform Collective Medical and analytics tool Co-Pilot in 2020, post-acute care management company QuickMar in 2019, and post-acute care-focused COMS Interactive in 2018.
